Primary Care Physician & Chief Medical Officer

John Mielke, M.D. John Mielke, M.D.

As Chief Medical Officer for Optage home and community services, Dr. Mielke oversees all of its clinical programs including House Calls, In-home Care & Services, and Hospice.

Dr. Mielke brings to House Calls a unique blend of experience in geriatric health care, clinical management and medical direction in long-term and sub-acute care. He has significant depth in health care quality, physician/patient and family relationships, geriatric health management, innovation and patient safety. “In this environment of rapid change in health care, we are pleased to provide personalized services to older adults in their homes to help them achieve their health care goals,” he said.

After receiving his medical degree in 1979 from the University of Minnesota Medical School, Dr. Mielke completed his residency in 1983 at Abbott Northwestern Hospital. He is certified in internal medicine and medical direction with qualifications in geriatrics. Dr. Mielke's memberships include: the American College of Physicians, the American Geriatrics Society, and the American Medical Directors' Association.

Practice Administrator

Mary Bullock, CNP Mary Bullock, CNP

Mary brings direct care, providing examinations and assessments, treatment plans, and education about wellness, disease prevention and chronic disease management.

From her wealth of experience in geriatric health care, nursing management, clinical research and long-term and sub-acute nursing care, Mary has gained a clear focus on how best to assist older adults to achieve their optimum potential as they age. "I enjoy listening to and learning from my clients' experiences," she said. "I want to support them and their families through comprehensive primary care within a context of the Christian values of Optage."

Along with her extensive experience, Mary holds a Master of Science-Nursing/Family Nursing Practitioner degree from Metro State University in St. Paul, MN and she graduated cum laude from Bethel University in Arden Hills, MN with a Bachelor of Science in Nursing.

Certified Nurse Practitioner

Jennifer Dart, CNP Jennifer Dart, CNP

Jennifer provides care for older adults in the comfort of their homes, through examinations and assessments, treatment plans, and education about wellness, disease prevention and chronic disease management.

With extensive experience in primary care management for older adults with complex and chronic medical conditions, Jennifer is a valuable asset to her patients and the Optage House Calls team. Additional expertise in geriatric health care, dementia care, and advanced care planning in a variety of settings further prepared her for this role.

Jennifer holds a Master of Science-Nursing degree from St. Louis University and she graduated cum laude from the University of Wisconsin-Eau Claire with a Bachelor of Science in Nursing. She is a member of the Gerontological Advance Practice Nurses Association and the Minnesota Medical Directors Association.

Certified Nurse Practitioner

Lowie Sipprell, CNP Lowie Sipprell, CNP

Lowie provides care for older adults in the comfort of their homes, through examinations and assessments, treatment plans, and education about wellness, disease prevention and chronic disease management.

With extensive experience in primary care management for older adults with complex and chronic medical conditions, Lowie is a valuable asset to her patients and the Optage House Calls team. Additional expertise in geriatric health care and total care management in a variety of settings further prepared her for this role. She considers serving within Presbyterian Homes & Services' Christian culture a tremendous privilege.

Lowie earned her Master of Science degree in Nursing and Nurse Practitioner from St. Catherine University in St. Paul, Minnesota. She graduated from the University of North Dakota with a Bachelor of Science in Nursing.

Gerontological Nurse Practitioner

Lisa TerHaar, CNP Lisa TerHaar, CNP

Lisa brings direct care to older adults in the comfort of their homes, providing examinations and assessments, treatment plans, and education about wellness, disease prevention and chronic disease management.

With expertise in medical management and care coordination for persons with acute and chronic conditions in long-term care, transitional care and assisted living settings, Lisa has much to offer her patients and the Optage® House Calls team.

Lisa received her Master of Science-Nursing degree from the University of Minnesota and was a recipient of the Allina Senior Care Geriatric Scholarship. She graduated from Calvin College in Grand Rapids, Michigan with a Bachelor of Science in Nursing. She is actively certified as a Gerontological Nurse Practitioner and a member of the American Nurses Association.

Care Manager

Caroline Kennedy, LSW Caroline Kennedy, LSW

Caroline seeks to optimize the quality of life for older adults by discussing their needs and finding the available and appropriate services to develop, implement and support a care plan for them.

One particular benefit to her clients is Caroline's expertise in geriatric care management and coordination in a variety of settings. Additionally, she serves them and the Optage House Calls team by drawing on her experience in community services, social services and marketing.

Caroline received her Bachelors degree in Human Services/Gerontology from Metropolitan State University in St. Paul, Minnesota and completed a second major in Social Work at the College of St. Catherine (now St. Catherine University), also in St. Paul. She is currently licensed in Minnesota as a Social Worker.

Primary Care Physician

Margaret Rice, MD Margaret Rice, MD

As the primary care physician for Optage® home and community services, Dr. Margaret Rice oversees administrative and clinical support for all clients in collaboration with the other members of the House Calls team.

Dr. Rice's distinctive experience in health care includes clinical services in senior housing such as transitional care and skilled nursing care centers. She brings her knowledge and skills in medical direction for older adults, along with certification in hospice and palliative care, while maintaining a strong interest in both osteoporosis prevention and chronic disease management.

Dr. Rice received her medical degree in 1982 from the University of Minnesota School of Medicine and completed her residency in 1985 in internal medicine at the University of Minnesota Hospitals in Minneapolis. She holds a master's degree in chemistry and a bachelor's degree in biology. Dr. Rice's memberships include: the American Geriatric Society, the American College of Physicians, the Hennepin County Medical Society and the Minnesota Medical Director's Association.

Judy Palmer, CNP Judy Palmer, CNP

As a Nurse Practitioner, Judy Palmer-Nessinger provides care for older adults in the comfort of their homes, through examinations and assessments, treatment plans, and education about wellness, disease prevention and chronic disease management.

With extensive experience in primary care management for older  adults with complex and chronic medical conditions, Judy is a valuable asset to her patients and the Optage House Calls team. Additional expertise in geriatric health care in assisted living, long-term care and acute care further prepared her for this role.

Judy earned her Master and Bachelor of Science degrees in Nursing from the Metropolitan State University in St. Paul, Minnesota. She is actively certified as an Adult Nurse Practitioner and is a member of the American Academy of Nurse Practitioners.

Sherol Holck, RN, CNP Sherol Holck, RN, CNP

As a Nurse Practitioner for Optage® House Calls, Sherol Holck provides care for older adults in the comfort of their homes, through examinations and assessments, treatment plans, and education about wellness, disease prevention and chronic disease management.

With extensive experience in primary care management for older adults with complex and chronic medical conditions, Sherol is a valuable asset to her patients and the Optage House Calls team. Additional expertise in geriatric health care in assisted living, long-term care and acute care further prepared her for this role.

Sherol earned her Master of Science degree in Nursing from St. Catherine University in St. Paul, Minnesota. She completed her Bachelor of Arts degree in Nursing at Metropolitan State University, also in St. Paul. Sherol received her Registered Nurse diploma at Abbott Northwestern School of Nursing in Minneapolis, Minnesota. She is actively certified as a Gerontological Nurse Practitioner.

Kari Garin, RN, CNP Kari Garin, RN, CNP

As a Nurse Practitioner for Optage® House Calls, Kari Garin provides care for older adults in the comfort of their home through examinations and assessments, treatment plans, and education about wellness, disease prevention, and chronic disease management.

With extensive experience in management of diabetes, hypertension, obstructive airway disease, and coronary artery disease, Kari is a valuable asset to her patients and the Optage House Calls team. Her interest in gerontological health promotion, collaborative thinking, and strong teaching skills further prepared her for this role.

Kari holds a Master of Science in Nursing degree from St. Catherine University in St. Paul, Minnesota, and she graduated magna cum laude from the University of Wisconsin-Eau Claire with a Bachelor of

Science in Nursing. Kari is a member of the Gerontological Advance Practice Nurses Association and the American Association of Nurse Practitioners.

Florence Fon, RN, CNP Florence Fon, RN, CNP

Florence provides care for older adults in the comfort of their home through examinations and assessments, treatment plans, and education about wellness, disease prevention, and chronic disease management.

With extensive experience in care for older adults living with acute or chronic conditions, Florence is a valuable asset to her patients and the Optage House Calls team. Her expertise in health maintenance, diabetic care and dietary management, along with a demonstrated ability to work with members of other disciplines to enhance patient care, further prepared her for this role.

Florence holds a Master of Science in Nursing degree from Walden University in Minneapolis, Minnesota and received her Bachelor of Science degree in Nursing from Grandview University, Des Moines, Iowa. Florence is a certified brain injury rehabilitation specialist and member of the American Association of Nurse Practitioners.

Patricia Henjum, CNP Patricia Henjum, CNP

Patricia provides direct care for older adults in the comfort of their homes through examinations and assessments, treatment plans, and education about wellness, disease prevention, and chronic disease management.

She is a valuable asset to her patients and the Optage® House Calls team, bringing over three decades of clinical experience, including 20 years caring for older adults living with medically complex health conditions. Her dedicated care for older adults living in assisted living, memory care and long-term care settings, along with her effective collaboration within multidisciplinary teams, further prepared her for this role.

Patricia holds a Master of Science in Nursing degree from the University of Minnesota and received her Bachelor of Science degree in Nursing from the College of St. Benedict, St. Joseph, Minnesota. Patricia is Board Certified as a Gerontological Nurse Practitioner by the American Nurses Credentialing Center.

Patty Coulter, ANP-BC Patty Coulter, ANP-BC

Patty Coulter provides direct care for older adults in the comfort of their homes through examinations and assessments, treatment plans, and education about wellness, disease prevention, and chronic disease management.

Patty is a valuable member of the Optage® House Calls team who brings over 20 years of clinical experience in a variety of settings, including acute care in ICU/CCU, emergency care, assisted living and skilled nursing care. Throughout her career, Patty has demonstrated her collaborative skills within a multi-disciplinary team resulting in the best possible patient outcomes.

Patty holds a Master of Science degree in Nursing from George Mason University, Farifax, Va., and received her Bachelor of Science degree in Nursing from California State University, Dominguez Hills, Calif. She also holds an Associate Degree in Nursing from Antelope Valley College, Lancaster, Calif. She is Board Certified as an Adult Nurse Practitioner by the American Nurses Credentialing Center. She has also been certified by the American Association of Nurse Practitioners and the American Heart Association.

Office Staff

During regular business hours (Monday through Friday from 8:30 AM to 4:30 PM) our physician and nurse practitioners are "out in the field" visiting patients. Our office staff of competent, medical associates are available for:
